Who’s on Your Medication Management Team?

Ask any caregiver about the challenges of medication management and you’ll get an earful. All too often, we hear about older adults who take their best shot at organizing and tracking medications, but fall short in execution. The easiest solution is one that’s all too frequently overlooked – put together a winning team.

The medication management dream team may be right under your nose. It may require no additional resources – just making sure that each team member knows their role. To use a sports analogy, you may already have an assistant coach in place if you have a nonmedical caregiver visiting, providing medication reminders and asking questions to ensure that the patient understands which medications are to be taken when. They may also alert the appropriate party when they notice that there’s potential for a medication to run out, if they observe that the patient is having difficulty with medication side effects.

Who’s the head coach of the medication management team?  Frequently this role is taken on by a spouse or an adult child – and sometimes the responsibilities of this important job are split. Medication procurement, organizing medications (perhaps filling a pill box?) and refill management are among the most important duties this job requires. When we consider that older adults often takes multiple medications, multiple times a day, sometimes prescribed by multiple doctors – and maybe filled by multiple pharmacies – we have a lot of multiples at play.
